Qualifications:
**Activity Director Certified (ADC) Required**
- Requiresknowledge of the skills necessary to conduct assessments of the history of eachresident's past experiences and activities, or those that the resident might beinterested in learning or participating in
- Possessedskills to interview residents, and communicate with community resources. Musthave knowledge of community agencies and other resources for making referralsfor outside activity interests that meet the resident's personal needs
- Requiresability to communicate effectively with staff, residents and families. Must beable to plan, implement and work with only minimal direct supervision. Must beable to write clearly and concisely in activity assessments and progress notes.Must be a good listener and be able to display understanding and a helpfulattitude towards the problems and concerns of the residents
- Must knowand uphold Resident Rights and State/Federal Regulations
- Allapplicants will undergo a drug screen, criminal history check, and history andphysical as part of the interview process
Duties and Responsibilities:
- Overseeseach resident's activity needs and ensures the development of an activityprogram that meets the needs and interests of the residents
- Eachresident will have an activity history assessment sheet completed upon admissionthat reflects the resident's needs and interests
- Eachresident's care plan will reflect the interests indicated on their MDS, if theActivity section triggers a problem and will be written every 90 days or moreoften if needed
- Eachresident's activity progress note reflects the progress made towards activitygoals and any other significant developments on their care plan every 90 daysor more often as needed
- Staff willdevelop and post a calendar of activity events in each health care room andnurses station by the first of each month
- Visits eachresident upon admission to obtain an activity history and to assess theresident's needs within 72 hours, as needed
- Visit withthe resident's spouse, surrogates, or other appropriate individuals, as needed,to obtain information about past and present family history and previouspersonality patterns, which are incorporated into the activity assessment andgoals
- Attend CarePlan Conferences as needed
- Visitresidents on a regular basis to assess needs and progress
- Maintainconfidentiality regarding information obtained during interviews andconversations with resident and family unless the health and well-being of theresident is affected
- Assists theresident in adjusting to the facility and promote a positive environment forthe continuity of relationships with family and friends
- Adhere toResident Rights and resident dignity
- Ensurehome-like environment, dignity, control, and privacy for residents. Encourageresidents to have their own belongings, i.e. pictures, bedding, furniture, etc
- Assess andmeet needs for emotional support. Encourage residents, as needed, and informSocial Services or nursing staff when additional support is needed
- Encourageresidents to continue to have contact organizations, churches, and otheractivities enjoyed prior to admission
- Assistresident, spouse, and family to utilize community resources
- Assistresidents in finding desired social/activity within the community not offeredwithin the facility, and encourage participation in family education/support groupmeetings
- Writedischarge plan for the activity section on each "Home Discharge Plan" for theresident being discharged from the facility
- Writeactivity instructions on each resident being discharged from the facility, andsign and date entries before the discharge
Performsrelated tasks as required:
- Helps withfacility parties
- Completespurchase requisitions for department supervisor to approve
- Supervisesvolunteers involved in department activities
- Attendsstaff meetings and facility in-services, as required
- Maintainsgood working relations with staff in all departments
