The Benefits Specialist Chester provides supportive services including outreach, monitoring, assessment and service planning, support network building, consultation to families, side-by-side skill training and instruction in the use of community resources, including referrals to community assistance as needed.
Position Requirements

Provide service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team (ITT); assume responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals in collaboration with the client and team; provide individual and supportive therapy and symptom management.

Assess clients' eligibility for entitlements (Social Security, Medical Assistance, food stamps, cash assistance, transportation, housing assistance, etc.).

Assist and support client to carry out necessary steps in order to obtain and keep ongoing entitlements (i.e. supporting individual through application process, providing transportation, providing information on riding the bus, assisting with obtaining necessary documents to expedite approval).

Position Requirements Continued

Ensure clients have adequate financial support (e.g., gain employment or apply for entitlements).

Teach money management skills and assist clients in accessing financial services.

Help clients' access reliable transportation.

Provide side-by-side support and coaching to help clients socialize (e.g, going with a client to a basketball game).

Plan, structure, and prompt social and leisure-time activities on evenings, weekends, and holidays.

Participate in client-centered comprehensive assessment of psychiatric history, mental status, physical health, use of drugs or alcohol, education and employment, social development and functioning, activities of daily living, and family structure and relationships.

Provide on-call crisis interventions and serve as back up after hours on rotating schedule.

Agency Requirements

Follows policies and procedures of the agency, including administrative, clinical, quality assurance and personnel.

Maintains good attendance (daily, meetings and other assigned tasks).

Maintains timely documentation of all work assignments and unusual incidents for risk management to be reported within 24 hours of the incident.

Routinely keeps supervisor informed about attendance and job assignments.

Demonstrate customer friendly approach and work cooperatively with all clients, family members, peers, other employees and referral sources.

Complete all mandatory staff development trainings.

Qualifications/Requirements

Education/Training: A Bachelor's degree with major course work in criminal justice sociology, social work, psychology, counseling or education; and two years direct service experience with mental health. Child Abuse and Criminal Clearance required.

Job Knowledge: Understanding of principles of mental illness and ability to implement them, and behavioral best practice and crisis management planning.
